Nucleus Sports Academy Inc. must be made aware of all medical conditions of participants at the time of registration for a course/program or event.
Nucleus Sports Academy Inc., reserves the right to cancel or alter any classes, times, costs or locations without notice as required due to low enrolment, change of policy, or availability of facilities or coaches. Full refunds will be issued for course/program/event cancellations only.
Registration is complete only when the full payment has been received. Refunds will be granted when refund/withdrawal request is made no less than 24 hours prior to the start of a course/program/event. An administrative fee of $25 will be deducted from all refunds. Refunds will not be issued when refund/withdrawal request is made on or after the start date of the course/program/event.
Players are to attend all days/sessions for the course/program/event in which they are registered. Make-up classes, refunds and/or credits will not be provided in the event that participant misses a scheduled day/session during the course/program/event.
I give permission for me (participant) or for my son/daughter (if participant is under the age of 18) to be a model for Nucleus Sports Academy Inc., and I hereby assign all rights of the film/photography/videotape/sound recordings to Nucleus Sports Academy Inc.. I authorize the use of the same by Nucleus Sports Academy Inc. and those acting with its permission for the purpose to use, reuse, and/or broadcast and republish this photograph/videotape recording. Persons (not acting on behalf of Nucleus Sports Academy Inc.) are not permitted to take photographs and/or videotape recordings of Nucleus Sports Academy Inc. events and/or participants (during sessions).
I give permission for Nucleus Sports Academy Inc., to send me electronic messages for Nucleus Sports Academy Inc., reasons. I understand Nucleus Sports Academy Inc. will not sell or rent personal information to third parties. I also understand that if I no longer wish to receive further information or to be contacted by Nucleus Sports Academy Inc. I can email admin@nucleussportsacademy.com.
